A Caesarean (C-section) delivery is a major abdominal surgery, and although most women recover well, some may develop complications involving the abdominal wall. One such possibility is an incision hernia, which can occur when tissue pushes through a weakened area around a previous surgical incision.
A hernia may not appear immediately after a Caesarean section. How Can a Hernia Develop After a Caesarean (C-section) Delivery?
During a Caesarean section, an incision is made through the abdominal wall to reach the uterus. The tissues are carefully closed after delivery, but the area may remain structurally weaker than the surrounding abdominal wall.
If the muscles and connective tissues do not regain sufficient strength, abdominal pressure can cause tissue to push through the weakened area. This can result in an incision hernia.

Recognizing the Signs
A noticeable bulge near or around the Caesarean scar is one of the most common signs. The swelling may become more prominent when standing, coughing, sneezing or straining and may become less noticeable when lying down.
Some women may experience discomfort, pressure, pulling or pain around the affected area. However, a hernia can sometimes cause little or no discomfort.
Factors That May Increase the Risk
Several factors can contribute to the development of an incision hernia after abdominal surgery. These may include obesity, wound infection, poor wound healing, smoking, increased abdominal pressure and certain conditions that affect connective tissue.
Repeated pregnancies or activities that place significant strain on the abdominal wall may also contribute to symptoms in some women.
Not Every Post-Caesarean Bulge Is a Hernia
A lump or change in the appearance of the abdominal area after a Caesarean delivery does not automatically mean that a hernia is present. Other conditions, including normal changes in abdominal tissues, scar-related changes or separation of the abdominal muscles, can produce similar appearances. A clinical examination is therefore important for identifying the actual cause.
Diagnosis and Medical Evaluation
A doctor may diagnose an abdominal wall hernia through a physical examination, particularly by assessing the swelling while the patient stands, coughs or strains. If the diagnosis is uncertain or the hernia needs further assessment, imaging such as an ultrasound or CT scan may be recommended depending on the individual situation.
Treatment Depends on the Hernia
Small hernias that cause no symptoms may sometimes be monitored based on a doctor’s assessment. However, a hernia that is painful, enlarging, affecting daily activities or associated with complications may require surgical treatment.
Hernia repair generally involves returning the protruding tissue to its appropriate position and strengthening the weakened abdominal wall. The specific surgical approach depends on factors such as hernia size, location, previous surgery and the patient’s overall health.
Warning Signs That Need Urgent Attention
A hernia that suddenly becomes very painful, firm or difficult to push back may require urgent medical assessment. Severe abdominal pain, vomiting, abdominal swelling or difficulty passing stool or gas can also be warning signs of complications such as bowel obstruction or strangulation. These symptoms should not be ignored and require prompt medical attention.
Getting the Right Assessment After Caesarean Surgery
A new or persistent bulge near a Caesarean scar should be evaluated rather than assumed to be a normal part of recovery. Early assessment can help determine whether the swelling is related to an incision hernia or another abdominal wall condition.
